P ost Offer Pre-Employment Physicals:

A. POET (Post-Offer Employment Testing): The post-offer comprehensive test is administered after application, interview and hire. The test may not be performed until the employer has made a conditional offer and must be completed before the individual begins work. This is where you can collect medical information. This test is a 45min to 1-hour test with medical measurements, maximum effort lifting and essential function tests. This test will gather baseline data to determine a candidate’s ability to perform the demands of the job.

Post-Employment Testing:

B. FFD (Fitness-for-Duty Examination): Post-employment testing in response to a job-related injury and/or employee inability to perform a job adequately. This category also includes periodic testing for employees in position affecting public safety (i.e. firefighters, police officers).

  • If the employer observes objective, reliable evidence of performance issues indicating that the employee may not be able to perform essential job functions or may pose a direct threat to the health and safety of the employee or others because of the “condition”.
  • When an employer has a reasonable belief that an employee’s ability to perform necessary job functions will be hindered by a medical condition or that he/she will pose a direct threat due to a medical condition.
  • When an employee wants to return to work after being on leave for a medical reason.
  • When an employee requests a reasonable accommodation.
  • When the employer requires periodic testing and monitoring of employees in positions affecting public safety.
  • When an employer requests medical examination required by federal law or regulation.
  • When an employee voluntarily participated in a wellness program (cancer detection, screening for BP, etc).

FFD: Testing process

  1. Initial process – Consent and personal ID information
  2. Medical History Interview
  3. Pre-Test HR/BP only when indicated
  4. Musculoskeletal measurements – Correlate directly to the injury or illness that triggered test
  5. Dynamic Lifting – Perform only the lift postures that are qualifying criteria for the position
  6. Job Specific Tasks – Perform all the job-specific tasks for which the client/position qualifies

C. Upper Quadrant Testing: The UQ testing is a complex medical test that focuses on upper extremities and cervical regions. However, it also offers a physical demand criteria. The goal of the UQ testing is to identify post-offer candidates that may have pre-existing signs and symptoms that need further evaluation by a medical physician, for they could be at risk in the performance of repetitive upper extremity tasks. This may also be used for testing an FFD candidate with an upper quadrant injury.

  5. Static Testing - grip and pinch measurements

  2. Dynamic Lifting - all 4 standard postures (floor to knuckle, 12” to knuckle, knuckle to shoulder, shoulder to overhead)

D. Physical Agility Testing: Post-employment does not always include medical measures. However, can focus on the physical demands testing required for the employment position (dynamic lifts and job specific tasks). Employers may request this type of testing for all employees in a particular job class or part of their periodic monitoring to ensure that employees remain capable of their demanding tasks. This test may also be used when an employee is on leave ad their provider suspects that there is no legally compliant reason to collect medical measurements (ie - off work due to family member illness).

  • Dynamic Lifting - perform only lifting postures that are for the employment position

E. FCE (Functional Capacity Evaluation): The FCE is a medical test to measure an employee’s function post-injury. Data collection protocols mirror the functional employment testing closely, but the clinician has the flexibility to modify tests and add any additional tests indicated to fully evaluate the patient’s condition and functional abilities.

How do you know when a patient is providing their best effort during testing to determine if they are physically capable of safely returning to work? Simple – through XRTS Sincerity of Effort Validity Testing.

XRTS FCE: Legally-defensible XRTS Testing provides empirical, objective analysis of a patient’s effort. Through a series of distraction-based checks and balances we can determine a patient’s true lifting, gripping and strength measures. There are two components of the XRTS test, a Hand Strength Assessment and a Lifting Assessment. Both rely on distraction-based methodology. For example, a patient is tested to see how much force can be produced with each hand using standard testing gauges. The data from repeated measures and the simultaneous testing of both hands is analyzed by applying seven statistically-based criteria and can show if there is excessive variation that might indicate less than sincere effort.

F. Job Analysis:

Job Analysis is the most important step of a comprehensive injury management program. The job analysis is the basis for determining the essential functions of the work and for developing testing to determine the physical capabilities of your workforce. It provides objective support that can be crucial in answering any ADA, EEOC or FMLA claims. It provides protection to employers in cases of a claim of disability by having the essential functions clearly defined. The Job Site Analysis is also a useful post-injury tool in determining safe return to work criteria following rehabilitation as well as establishing for comparison with the results of a Functional Capacity Assessment.

What Does Creating A Job Analysis Look Like?

Aspects of A Job Analysis:

  • This consists of a Work Steps representative visiting the job site to perform hands-on evaluations of the physical requirements of the employees.
  • This entails physically measuring/weighing objects, photographing body positions, observing equipment, and how it’s being performed by the employees. It might require filming workers to make the appropriate recommendations that would alleviate traumas and prevent injuries.
  • The ADA specifically addresses the necessity of a job analysis to identify essential functions of a position, and each category the employee will be tested on. Being able to offer a representative to observe the workers while performing tasks and being present at the job site to perform the hands-on evaluation gives quantitative validly. This includes a fair sample of the tasks necessary for a successful job performance.
  • A job analysis gives content validity to the testing process for future employees, by establishing valid job descriptions and subsequent testing that are consistent with business necessity.
